When President Obama signed the Tobacco Control Act into law on June 22, 2009, he called attention to the importance of smoking cessation in Americans. Smoking results in more than 400,000 deaths annually in the United States from cancer, emphysema and other respiratory illnesses, cardiovascular disease, and spontaneous abortions or miscarriages during pregnancy. In the United States, there is an increased risk for premature birth in the fetus of pregnant women if the mother smokes, as well as growth retardation of the fetus. Over 1,000 infants die annually because the mother smokes.

On June 22, 2009 President Obama signed a tobacco control act which allows the Food and Drug Administration (FDA) to regulate certain types of cigarettes. This will be helpful in improving tobacco avoidance in children and in adults. It will, in addition, be an important path for discussions between parents and their children, discussions between physicians and their patients, and discussions within families about the helpfulness of tobacco control.

I have continuously stressed that you should be screened for serious illnesses. This includes heart disease, bone disease, diabetes, as well as cancer. Cancer screening tests are very well defined, and you probably have already had some of them if you are over the age of 40 to 50. Screening for serious chronic disorders can allow your doctors to diagnose these diseases earlier when there has been less damage to organs, and this can preserve your quality of life.

It would seem obvious that having health insurance helps to provide better access to doctors and provide better treatments for serious illnesses. However, since many patients without health insurance can access urgent care clinics, emergency rooms, attend health clinics, and obtain primary care for cash, it is important to look at the impact of health insurance on control of important illness, and the relative impact of health insurance on different races and ethnicity.

We are undergoing fundamental and profound changes in healthcare in the United States. In order to make the care within a hospital more efficient, and discharge patients in a shorter length of time, many hospitals have changed the physicians who are responsible for the care of the patient while in the hospital. A group of physicians trained to give inpatient care, but not outpatient care, has developed and they are called “hospitalists.”

Pregnancy can be one of the most rewarding experiences in a woman’s (and man’s) life. However, many pregnancies are not successful. A miscarriage, a failure of a woman to deliver a healthy baby, is common. Miscarriage occurs in 15 to 20 percent of women who know they are pregnant, usually before the 13th week of pregnancy. However, most experts recognize that the true frequency of miscarriage is probably about 50 percent since many pregnancies spontaneously fail even before a second period is missed.
